
Orange juice can cause or worsen diarrhea in some people. Research suggests several possible reasons, including incomplete absorption of sugars such as fructose, particularly when larger amounts are consumed. Studies of orange juice itself have also found changes in bowel frequency and gastrointestinal symptoms.

Among more than 400 users with relevant diary data, orange juice ranks #3 among foods associated with worse diarrhea in the 3 hours after consumption. Orange juice and diarrhea also ranks #45 among all food–symptom pairs tracked in the app.
Individual responses vary, however, and many people tolerate orange juice without problems. Orange and oranges are logged separately from orange juice.
One possible explanation is incomplete absorption of carbohydrates such as fructose.
When fructose isn't completely absorbed in the small intestine, it can increase the osmotic load in the bowel and subsequently be fermented by intestinal bacteria. In susceptible people, carbohydrate malabsorption can contribute to abdominal discomfort, gas and diarrhea.[1][2]
Experimental evidence shows that the amount consumed matters. Ravich and colleagues gave healthy volunteers fructose solutions and found incomplete absorption in 6 of 16 people after a 50 g dose. Five of those six experienced cramps, diarrhea, or both. Fructose malabsorption was related to both dose and concentration.[3]
A later dose-response study found a similar pattern. Healthy volunteers generally absorbed 15 g of fructose, while malabsorption became substantially more common at larger doses.[4]
This makes fructose malabsorption one plausible explanation for why orange juice could cause diarrhea in some people. But orange juice is not the same as drinking a large dose of fructose on its own.

Fructose absorption depends partly on the other sugars consumed with it. In particular, glucose can facilitate fructose absorption, so foods containing similar amounts of glucose and fructose may be better tolerated than foods containing a large excess of fructose.[1][2]
Orange juice contains both glucose and fructose rather than being a concentrated source of free fructose alone.
This is important because some of the strongest experimental evidence for fructose-related diarrhea comes from large doses of isolated fructose, rather than ordinary servings of orange juice.[3][4]
So orange juice containing fructose does not automatically mean it will cause diarrhea. The amount consumed and individual differences in digestion are likely to matter.
Studies of orange juice itself provide useful evidence.
In an eight-week clinical trial, 48 healthy adults consumed 500 mL of natural orange juice every day, either alone or enriched with the fructan fibre levan. Researchers monitored bowel movements, stool consistency and gastrointestinal symptoms. Both orange-juice groups experienced an increase in stool frequency, as well as increased bloating scores.[5]
This suggests that a relatively large daily amount of orange juice can affect bowel habits, although the study does not show that orange juice routinely causes significant diarrhea.
Another randomized study involved 192 healthy women who drank 400 mL of orange juice daily for 21 days, either ordinary juice or juice enriched with fibre. Mild gastrointestinal symptoms including bloating and intestinal rumbling increased, but intense gastrointestinal adverse effects were not reported.[6]
Together, these studies suggest a useful middle ground: orange juice can affect gastrointestinal function, but many healthy people tolerate fairly substantial amounts without severe symptoms.
That leaves plenty of room for individual differences — which is also what we see in real-world food and symptom diaries.

These are observational diary data, so the association does not by itself prove that orange juice caused the diarrhea. But combined with research showing that fruit sugars and orange juice can affect bowel function, it provides a good reason to consider orange juice as a possible trigger if you notice symptoms after drinking it.

Fructose absorption is dose-dependent. Experimental studies show that larger fructose loads are more likely to be incompletely absorbed than smaller ones.[3][4]
The studies that detected gastrointestinal changes from orange juice also used fairly substantial quantities — 400–500 mL per day.[5][6]
A small glass therefore shouldn't automatically be treated as equivalent to a large bottle of juice.
This may also explain why someone can drink orange juice without problems on one occasion but experience diarrhea on another. Portion size, other foods eaten at the same time and individual digestive differences can all potentially affect the response.
If you're trying to determine whether orange juice affects you, it can therefore be useful to record how much you drink, not just whether you drank it.
Whole oranges and orange juice aren't identical.
Whole fruit retains its natural structure and fibre and is generally eaten more slowly. Juice makes it easy to consume the sugars from several oranges relatively quickly.
Those differences provide plausible reasons why someone might tolerate whole oranges differently from orange juice.
However, there isn't strong clinical evidence showing that replacing orange juice with an equivalent amount of whole orange specifically prevents diarrhea. If you suspect orange juice is a trigger, treating whole oranges and orange juice as separate foods when tracking symptoms may be more informative than assuming they will affect you in the same way. The Eat Smart Kiwi association appears during the 3 hours after orange juice consumption.
That timeframe is plausible for gastrointestinal symptoms, although the exact timing varies between people and depends on the mechanism involved. Poorly absorbed carbohydrates can affect water movement in the intestine and undergo bacterial fermentation.[1][2]
A single episode of diarrhea three hours after drinking orange juice doesn't establish that the juice caused it. A repeated pattern across multiple occasions is much more informative.
If you already have diarrhea, drinking large amounts of fruit juice may make symptoms worse for some people. Reviews of fructose and IBS dietary management note that poorly absorbed sugars can contribute to diarrhea and abdominal cramping.[1][2]
That doesn't mean everyone with diarrhea needs to avoid orange juice. If you tolerate it without any noticeable worsening of symptoms, there may be no reason to remove it solely because someone else finds it problematic.
If you think orange juice causes diarrhea, look for a repeatable relationship rather than drawing a conclusion from one episode.
